Falls Prevention - Southport & Formby

The service supports people 65 years  and over, living at home, or in a residential/nursing home, who are functionally mobile and able to follow instructions. Falls related to alcohol or substance misuse should be directed to addition services.

Frailty Service - Southport and Formby

This Multidisciplinary Team (MDT) consists of Advanced Practitioners, Nurses and Therapists.

It  supports admission avoidance for frail, older people. 

Wheelchair Service - Southport and Formby

The Wheelchair Service serves the registered population of, Southport & Formby localities of Southport, Ainsdale and Formby.  The Wheelchair Service provides wheelchairs on a permanent loan basis to those whose indoor mobility is permanently restricted and will remain so for at least six months.
